Long Term Conditions Support Nurse

Long–term Support Nurses help patients and families to cope with long-term conditions, aiming to prevent unnecessary hospital admissions and GP visits. Our nurses will visit you in your own home to discuss your general health, medical history, current symptoms and specific problems you may be experiencing. Together we will work towards making your life easier and more manageable for you and your family.

A long term illness is defined as an illness that cannot be cured but can be managed through medication and/or therapy. There is no definitive list of long term conditions, COPD, diabetes, asthma, neurological conditions, organ failure and coronary heart disease are some which fall within this category.

Our Hospice Support Nurse is able to offer help and support to patients with a long term illnesses manage their condition, in their own home.


If you think any of these services could be of benefit to you, you may self-refer by telephone the Support Nurses using the contact numbers below. Referrals can also be made by any healthcare professional.

The support nurse will visit patients in their own home to discuss their general health and medical history. A full assessment will then be carried out to identify areas where the Hospice or other service can help.

You may need extra help in the home with personal care, your family may need help in supporting you, perhaps you would benefit from services within our day therapy unit, counselling support, etc.

You may be concerned about your Health, which you can discuss with the Support Nurse who will seek advice/support from other services, e.g. District Nurse, Physiotherapist, or give access to services the Hospice can help with.

Our aim is to help you live with your condition, in the best possible way, within youe home and to help to avoid/prevent (where possible) un-necessary admission to Hospital.
